If you change your name or move house, please notify
us as soon as you can.
We would also appreciate a phone number where we can
contact you if necessary; this will ensure that if you ever require urgent treatment
the doctor will know where to find you.

REGISTER
WITH THE PRACTICE?
Newborn baby - complete form FP58.
All other patients - complete your medical card.
Hand the completed documents in to the receptionist.
If you do not have the above documents, ask the receptionist for a form GMS1.
You will also be required to complete a short questionnaire.
You will receive a new medical card from Kent Primary Care Agency.
If you have not received your new card within six weeks, you should telephone 01622 655000.

Of Practitioner
The practice does not have personalised
lists and you will therefore be registered with the practice rather than a specific doctor.
