New Patient Registration
The doctors welcome new patients who live within our practice area.
Complete your medical card and hand the completed documents in to the receptionist.
- If you do not have the above documents, ask the receptionist for a form GMS1.
- You will also be required to complete a short questionnaire.
- You will receive a Statement of Registration from Kent Primary Care Agency. If you have not received one within six weeks, you should telephone 01622 655000
Preference Of Practitioner
The practice does not have personalised lists and you will therefore be registered with the practice rather than a specific doctor.
